Required Paperwork
All required documentation for athletes and parents to fill out in order to be eligible to compete in Constantine High School and Middle School athletics is now done online.
Both the student-athlete and the parent will need to create an account on https://constantinefalcons.com/.
You will need to:
- upload a photo of both sides of your completed athletic physical
- fill out the emergency release form
- agree to the athletic code of conduct
