Welcome to Constantine Public Schools, where we are committed to providing your child with the highest quality education. To ensure a smooth enrollment process, please follow these steps:
 
  1. Download Enrollment Forms: Take a few minutes to download the necessary enrollment forms below.
  2. Complete Forms: Once downloaded, please print out the forms and fill them out completely.
  3. Submit Forms: Deliver the completed forms to the school office your child will attend. If you have any questions while filling out the forms, please don't hesitate to contact the school secretary at the appropriate building.

* Do you live outside the school district? If so, you must seek approval first through our Schools of Choice program. Please complete a Schools of Choice Application.

Reminders:
      • Please print out each of the applicable forms
      • Please hand deliver (not mail) these forms to the school, along with:
          • Birth certificate
          • Proof of residency
          • Immunization records
          • Any other documentation you feel is helpful (i.e. IEP for Special Education students)
      • Additional paperwork is required for Special Education student enrollment
 
Building offices will open on Wednesday, July 31, 2024. It is recommended to phone ahead to assure that personnel will be available to assist you in registering your student(s).
