ALL middle school and high school student-athletes are REQUIRED to have a
sports physical prior to being allowed to practice beginning August 9th!
Constantine Public Schools and Covered Bridge Healthcare of Centreville are
teaming up to offer Sports Physicals to cover the 2021/2022 School year!
When: Wednesday, July 21st - 4:30pm until 8:30pm
OR Wednesday, July 28th - 4:30pm until 8:30pm
Where: Constantine High School - Physical Education Locker Rooms
(at the end of the hallway on the West side of the CHS Gym)
Cost: FREE!
By appointment only!
Sports Physical Sign-Up Sheets are in the CHS Office, which is open between
8am & 2pm Mondays through Thursdays
There are separate Sign-Up Sheets for boys and girls for each date &
each available time
*Each student-athlete MUST have an MHSAA Physical Form signed by
a parent or guardian BEFORE a physical can be performed!
Physical Forms are available on the desk in the CHS Office
between 8am & 2pm Mondays through Thursdays
If a student-athlete cannot make it to either of these days and times, Covered
Bridge Healthcare will offer a continuation of this free offering for sports
physicals at their location in Centreville for a limited time.
